Participant Waiver & Liability Agreement

I understand that while I am participating in the Montgomery Soccer Inc (MSI) tryout, there is a risk of injury. I understand that such an injury can range from minor to major injury. I hereby waive any and all claims, causes of action, right to entitlements, suits or damages against MSI, including any and all of its coaches, agents or representatives, as a result of or in conjunction with my participation during this tryout. I understand that participation in the MSI tryouts is not a guarantee of a position with a MSI team for any season and no implied guarantee of position has been made to me. I verify that the player candidate has no physical disabilities, impairments, or other medical conditions that will inhibit participation of soccer sport activities. I hereby accept and assume the risk of injury and understand the possible consequences of such injury.
